For younger patients, we will also require completed MINOR FORMS.

This will help Precision Health and your doctor understand your health history and your healthcare needs.

At your first appointment, your doctor will review your health history and determine if you are likely to benefit from spinal or soft tissue treatment. If you will be benefited, the Doctor will perform a thorough examination and assess the soft tissue, muskuloskeletal integrity, and neurological function.

The doctors will then review your case thoroughly and when you return for your follow up visit they will go over your findings in detail, what the treatment will be, and the fees associated with the services. If you decide to proceed with care, your first treatment will be administered that same day.
